   *Those who are unable to download the form are asked to contact our privacy officer via the inquiry form.
2. Fill in all required items on the Personal Information Disclosure Request Form.
3. Have identification documents ready.
• a) When the user is making the request: one (1) of the following.
  • ・A copy of both sides of your driver's license (valid, issued by the public safety commission of your prefecture. International driver's licenses are excluded.)
  • ・Copy of your My Number Card (only the side that does not show your identification number)
  • ・Copy of your health insurance card.
  • ・A copy of your Residence Card, Special Permanent Resident Certificate, or a Certificate of Alien Registration deemed equivalent to a Special Permanent Resident Certificate.
    *If your current address and the address on your identification documents differ, please attach a Certificate of Residence.
    *For information on permanent domicile included in the documents, any information other than the prefecture should be blacked out.
• b) When a request is made by proxy.
  • ・When making a request by proxy, provide a letter of attorney with the user's seal, a certification of official registration of the user's seal, and identification documents for the proxy (same as in "a" above).
  • ・For a legal representative, obtain documents verifying the right to act as a legal representative (family register or certificate of residence, etc.) and identification documents for the legal representative (same as in "a" above).
4. Fees and Payment Methods
• Requests for "Notification of the Purpose of Use" or "Disclosure"
  • ・¥1,000 (tax included) will be charged per request.
  • ・Once your request has been received, we will inform you of the account for the bank transfer.
  • ・Transfer fees will be borne by the person making the request.
    *Fees cannot be refunded even if we are unable to respond to your request for legal reasons.
5. How to Make a Request and Where to Send It
• a) When the user is making the request, use one of the following methods.
  • ・Email Attachment: Attach the Personal Information Disclosure Request Form and identification documents and send an email to our privacy officer.
    *Put [Personal Information] at the beginning of the subject line.
    *After we receive your email, we will inform you via email of the bank account where you will transfer the processing fee.
  • ・Simplified Registered Mail: Attach the Personal Information Disclosure Request Form and identification documents and send to our privacy officer via simplified registered mail.
    *After we receive your request, we will inform you via email or mail of the bank account where you will transfer the processing fee.
• b) When making a request by proxy, enclose the Personal Information Disclosure Request Form as well as identification documents for both the user and the proxy, then send to our privacy officer using simplified registered mail.
